The role:
As a District Nurse you will be working in partnership with patients and their families to deliver person centred, safe and effective care in the community.
You will work as part of the District Nursing team and have responsibility for a delegated caseload. The role will require you to undertake evaluation of patient care needs and ensure care plans are updated, and accurate at all times. Ensuring patients are able to remain independent in their own homes. You will liaise with all health care professionals and other agencies to ensure effective service delivery.

What you’ll need:
- Minimum of 6 months paid Community nursing experience
- Comply with NMC Professional registration
- Driving licence
- Enhanced DBS
