The role:
- The role of Community Psychiatric Nurse offers a unique blend of professional fulfilment and adaptable scheduling. You will provide vital healthcare services within the community, making a significate impact on patients’ lives.
- Working within the Community Mental Health Teams you will be providing robust assessment of patients (by telephone or face-to-face) who have been referred due to presenting with mental health difficulties.
- Identifying appropriate care pathways and ensuring the least restive option to meet the needs of the patient and ensuring their wellbeing.
- Signposting to appropriate care providers (including primary and tertiary care).
- Completing documentation and planning care for patients.
- Presenting cases to a Multi-Disciplinary Team for discussion around care pathways.
- Working hours – Monday to Friday 9am – 5pm
About the Trust:
The trusts provides inpatient and community-based mental health care across Bolton, Manchester, Salford, Trafford and Wigan. The Trust provides a wide range of specialist mental health and substance misuse services for patients from Greater Manchester and Beyond. They were rated good in their most recent CQC report.
What you’ll need:
A minimum of 6 months paid experience as a CPN
Comply with NMC Professional registration
Full UK driving licence
Enhanced DBS
