Community Clinical Nurse Manager 1

PE Global are now recruiting on behalf of our client based in Cork for a Community Intervention Team Lead Nurse. Full time – Day time hours with weekends rotated     

Duties

• To manage the CIT staff, their rotas and scheduling.
• Be the point of contact to support staff
• To accept CIT referrals, deciding what patients are eligible for the service, allocating the nurse and to deliver the care plan
• To ensure all patients are treated holistically by effective use of patient care planning including individual assessment/accurate evaluation, realistic planning and effective implementation.
• To oversee the preparation of reports to be given to /received from the staff and to ensure that the contents are communicated accurately and effectively as directed by the CIT Services Manager and Patient Flow Manager.
• To ensure that all the necessary nursing documentation relating to patients, are accurately maintained at all times and to ensure that patient confidentiality is fully respected.
• To assist the CIT Services Manager and Patient Flow Manager in ensuring that all nursing staff sign their full names to all necessary/relevant documentation

Candidate Requirements                                  

• Registered with the NMBI , general registration 
• Minimum of 5 years post registration experience
• previous staff management experience / person in charge experience
• Experience in Acute setting essential  
• Community Experience desired
• Full Driving License  
• Excellent communication skills and documentation skills 

High level of competency in the following areas for the Community Nurse:  

• Wound Care and IV Cannulation 
• Stoma care and patient education
• Diabetic care and patient education
• Palliative care and patient and family support  
• Phlebotomy sampling
• I.V antibiotic 
• PICC/PORT/Midline 
• Catheterisation (Supra pubic also preferable)
• PEG tubes and feeds
